Referral Requirements
If you are covered by an HMO plan, please note that prior approval (authorization) is required before your appointment. This applies to office visits, diagnostic testing, and procedures. Without insurance authorization, we will not be able to provide services in accordance with your insurance guidelines.
If approval has not been secured, we recommend contacting your primary care provider for assistance or visiting your nearest emergency department for urgent care needs.
